Terms of Membership
Your dues will be payroll deducted upfront monthly if One Year Commitment is selected. After one year, you will be on a month to month agreement. However, if you quit after completing one year, it will require another one year agreement to rejoin at the lower rate. If your employment is terminated for any reason, you are no longer eligible to be a member.
All members are required to complete a registration form and sign a liability waiver before using any of the equipment. A new security code will be issued each month. The center will be open from 5am until 11pm seven days a week and is subject to change at the discretion of the facility. (No one is allowed in the fitness center between the hours of 11:01pm and 4:59am) In order for everything to function properly we need to cooperate on an honor system. There will be a 20 minute limit on each cardio machine used, unless there is no one else waiting. Members should understand that due to circumstances beyond our control, there may be times in which equipment is inoperable.
EMPLOYEES & IMMEDIATE FAMILY MEMBERS ONLY
Immediate Family Members is defined as those living in the same household as the employee and is limited to spouse, child, parent, sibling. NO GUESTS ALLOWED. Anyone 12-15 years old must be accompanied by a parent – no one under the age of 12 allowed at any time.
Alcoholic beverages, illicit drugs, including anabolic steroids and/or tobacco, in any form are forbidden on the premises. It is strictly against Policy for Members to allow Non-Members access to the Security Code. Employees are responsible for the actions of family members. Any person found in violation of these Membership Terms will be denied further access to the Fitness Center as will be the Employee or Family Members of the individual violating policy.
LOCKER POLICY
Lockers are available for temporary or rental use while utilizing the facility. Long-Lewis Fitness Center will not provide locks. Locker rental is on a 1 year contract for an additional $5.00 a month or $50.00 a year if prepaid.
CLEANLINESS & FOOD AND DRINK POLICY
No food allowed in the fitness center. Members are responsible for themselves and their guests. Please let someone know of any spills promptly so it can be cleaned. There are no drinks allowed on the machines. Drinks should be left on the bar between the upper and lower level of the gym.
LIABILITY WAIVER
It is agreed that all use of the fitness center shall be undertaken by a member at his or her sole risk, and Long-Lewis Fitness Center shall not be liable for any injuries or damage to any member, or any property of any member, or subject to any claim, demand, injury or damages whatever, including without any limitations, those damages resulting from the negligent actions or failures to act on the part of Long-Lewis Fitness Center. The undersigned member hereby waives any such claim for injuries or damages. It is especially agreed that Long-Lewis Fitness Center shall not be responsible or liable to members for articles lost or stolen. It is also agreed by the undersigned member that he/she shall be responsible for any damages to the center facility, property or equipment.
AMENDMENT OF POLICIES
Long-Lewis Fitness Center reserves the right to change and/or add to these policies as it may be necessary for safe and proper management of the facilities and its programs.
