This form gives you an anonymous way to report anything that may be deemed illegal, unethical, immoral or anything related. If your claim leads to prosecution, you may qualify for a Cash Reward of $500 to $2,500.

When You Should Tell:

• If a co-worker is doing something that isn’t safe, not telling anyone could be a big mistake. Many cases of death, injury or environmental disaster have been caused or facilitated by such a “code of silence” mentality.
• If a co-worker is doing something unethical or illegal, your silence could cost a lot of money or get your company in serious legal trouble that may end up affecting your job.
• If a co-worker is sexually harassing or stalking another, you have an ethical obligation to help the victim by not protecting the offender.
• If a co-worker is embezzling or stealing in any way, not telling could make you an accomplice leading to criminal prosecution.

Don’t think of these and similar situations as tattling, but as Whistle-Blowing or doing what’s right by exposing what’s wrong.
